I don't know of a specific tool to this. We build custom automated migration tools, that could be used to move the data (rather easier) and the application code (rather harder). I don't of a specific tool to do this. We build custom, automated conversion tools to move the code and data. This is much more effective than manual recoding if you want the application to get converted reliably and/or you want it to continue doing what it did.
