Rolling Averages

Hi Guys

I need to calculate the rolling (moving average) of Cost by each doctor for 5 years on a 12 month period in order to smooth out the graph and add a trend line.

Guys here is my sample data:

Year Month Cost Doctor
2000 1 $20000 A
2000 2 $2111 B
.
.
2005 1 $cost A

For the doctors I just need to show a count of doctors for that time period.
